How Does The All-On-4-Concept Work?

All-on-4 was developed and perfected by Dr. Paulo Malo and his team of dentists and oral surgeons at the Malo Dental over a decade ago. It is a full arch treatment that replaces all the teeth in the upper and/or lower dental arches with a fixed bridge that is supported by 4 titanium implants.

In most cases, people only start to lose their teeth when the underlying jaw bone and connective tissue gets eaten away by bacteria. When too much bone loss has occurred, there won’t be any place available underneath the missing teeth to surgically insert a dental implant.

In such situations, patients will first have to go through a bone grafting procedure to replace the lost bone. A bone graft will take a couple of months to heal completely, and only then will you be able to get fitted with your replacement teeth.

With All-on-4, bone grafts are no longer necessary. It works around the bone loss problem by tilting the two posterior implants up to 45 degrees so that it can be properly supported by denser bones in the other parts of jaw or even the cheek bone. The remaining two anterior implants are inserted straight similar to a normal dental implant.

This dental implant technique has proven to be very effective for tens of thousands of patients in different parts of the world. What To Expect From The All-On-4-Procedure?

The initial evaluation consists in a comprehensive examination and diagnostic imaging. With the help of a specific computer software program is created a 3 dimensional image of your jaw. “Virtual implants” are placed in the 3D image to ensure accuracy at the time of your surgery. What are the different aspects of All-on-4?

All-on-4 gets its name from the four titanium implants that are surgically inserted into the upper and/or lower jaw to support a fixed bridge. It is a full arch replacement that allows patients to get a whole new set of upper and/or lower teeth, which can greatly improve their quality of life and give them back a perfect smile

The titanium dental implants used in the All-on-4 treatment were chosen not only because they are strong enough to last an entire lifetime, but also due to its versatility as far as surgical placement is concerned. The key to All-on-4 is the angle at which the two posterior implants are inserted into the jaw. By tilting it up to 45 degrees the implant can be place in areas where there is more bone density to support the bridge. Titanium implants will also eventually become part of the jaw as new bone growth directly attaches to its surface through a process called osseointegration, which translates to increased mechanical strength and stability.

What are the advantages of the All-on-4 treatment?

Since All-on-4 uses only 4 implants to support the bridge, it is less invasive than conventional dental implants which require more implants to be surgically inserted in the jaw.

When tooth loss occurs due to decay it usually means that the underlying bone and connective tissue supporting the tooth has also been eaten away by bacteria. Bone loss can also occur when teeth that are knocked loose due to external forces and are left untreated.

In most cases, patients with missing teeth will also have suffered enough bone loss in the jaw to make placing traditional dental implants impossible. To solve this, a separate procedure called a bone graft is required. For this procedure, bone tissue from another area or source is placed in the area where bone loss has occurred so that new bone tissue can grow around it. Only when the new bone has formed after several months can a dental implant be placed.

The main advantage of All-on-4, is that it can be performed on patients who have severe bone loss in the jaw without having to undergo a bone graft. With All-on-4, the two posterior implants are placed at an angle in areas of the jaw where there is more bone mass to properly support the bridge. This means you can get your implants and bridge fitted within the span of only a few days, versus several months or up to a year with conventional dental implants.

Typically, you can get your teeth extracted and have implants surgically inserted in a day. Our dentists will then place a temporary bridge on the same visit so no one will be able to tell that you’ve just gone through implant surgery and had all your teeth removed.
  • The temporary bridge is modelled specifically for you so that it fits and functions almost as well as the permanent bridge. While waiting for the implants to heal, the temporary bridge will allow you to eat normally as long as you avoid hard food. In the following days, there might be some instances of minor swelling or bleeding which is completely normal and tolerable even if you don’t take any painkillers or medication.
